[SLUG] Dead floppy drive?

Hi. Whenever I read data from the floppy drive, I get messages like
these:

Jan 26 00:14:12 pc kernel: end_request: I/O error, dev 02:00 (floppy), sector 5

and the reading process dies with "standard input: file: read failed
(Input/output error)". How can I debug this?

I have a USB floppy drive, with which I can read and write floppies and
lay down filesystems, but I can't make them bootable with lilo, and I
probably can't run fdformat. I had the bright idea of using a floppy
image instead and writing it to a floppy on another machine, but lilo
won't make that bootable either. Does it look like a hardware error?
What I haven't done is try booting from a floppy.
