Maybe /etc/hosts.{allow,deny} is disallowing it? Something like

ssh:ALL

in /etc/hosts.allow , I guess. The "service" part is the service as
listed in /etc/services .

I don't believe hosts_access logs anywhere, just silently closes the
connection.
