For the frontend you could use something as simple as the "dialog" utility. It has
some built-in functions to do forms, radio buttons, checkboxes, dropdowns, etc.. You
can then write it using something like this:

Another option for the front-end is KDE. If you install Kdevelop it has a bunch of
sample applications including one with forms and other user input.
