Some time ago I went through this with an ancient Thinkpad my daughter had.
It was with an ethernet card. The shape of the edges of the cards are such
that the 32 bit type will not seat fully when pushed into a slot designed for
a 16 bit card. Not knowing this at the time, I thought I had gotten a cheap
card that just didn't fit quite right and filed it down to make seat
correctly. Of course it didn't work (and fortunately I didn't blow anything
out!). I got the correct card and it worked.

Unfortunately, I don't remember the details well enough to say exactly how to
spot the difference.
