For most cases, Best Practical's Request Tracker seems to be good
enough. Or maybe overkill :) I know that the LAN airline from Chile uses
RT, with 60+ million cases and several hundred agents.

GLPI (and GLPI/OCS Inventory-NG) is good for IT platform ticketing, and
OTRS/OTRS::ITSM is good is you need ITIL support. I've heard of Mantis
and some others, but I've worked with RT, GLPI and OTRS.
