I use trac - it integrates with Subversion (and a bunch of others too),
and it has a wiki to discuss issues.

However, it is designed for software development, and, while easy to
use, doesn't have the "polish" that you need if your client's clients
are expected to see it. It works fine for a small company's /internal/
issues.

On Ubuntu server, do "apt-get install trac".

If they have an LDAP or AD already set up, you can do Authentication
against it using mod_authnz_ldap or mod_auth_kerb respectively, and the
user has the same username/password on the system automatically.

If you want to do trac and need help, shoot me an email - I want to do a
tutorial on setting it up for my web site anyway, and would love the
feedback.
