And it is the Sys Admin's responsibility to monitor and, if necessary,
punish those doing bad things. If an ISP has an incompetent admin,
RBL's take care of this, for my clients.

Exactly. After enough of the blocked IP's client's complain. The
doctor's (to use your doctor simile) the clients sue for malpractice and
move to someone more respectable. Your argument is jejune.
